Health Extension for Diabetes
Online, AL
Health Extension for Diabetes is a unique education and support program that connects participants to local clinical resources through partnerships with local hospitals, clinics, and healthcare providers. Through a series of 8 sessions provided over 4 months, participants receive education for living with diabetes as well as support from each other, trained extension agents, and local healthcare professionals. The objectives of the program are to improve participants’ knowledge of diabetes, behaviors associated with diabetes management, and diabetes-related health outcomes
