1. Health / Release Form (Students Only)

Health / Release Form (Spanish Version) - NOTE: This form may be used as REFERENCE ONLY. We must have all health forms turned in using the English form for the safety of all campers.

2. Youth Parent Consent Form (Students Only)

Youth Parent Consent Form (Spanish Version) - We WILL accept these forms in Spanish to ensure all parents are aware of the consent they provide while their child is at the Center.

3. Field Group Assignment Sheet (due 2 weeks prior to event) - All Participants
